Closes issue #90 (closed).

Ported "traditional" mid-point refinement based mesh generation from HHG to HyTeG for the Spherical Shell. Pipieline is okay, besides intel_19_eigen_petsc, but the latter seems to be an unrelated issue also currently showing up in master itself.
